About the Role

As a Clinical Case Manager, you will be the primary point of contact for patients and consultants throughout the cancer treatment journey. You’ll manage the end-to-end process—from onboarding new referrals to offboarding patients—ensuring treatment plans are accurate, compliant, and delivered seamlessly. This role is pivotal in coordinating care across multiple stakeholders, including pharmacists, nurses, schedulers, and external consultants.

Core Objectives

- Onboard new patients: Review clinical documentation, set up treatment plans, and conduct onboarding calls.
- Manage treatment plans: Monitor upcoming visits, prescriptions, pathology results, and ensure compliance with protocols.
- Coordinate stakeholders: Liaise with internal teams and external consultants to resolve queries and manage changes.
- Ensure regulatory compliance: Maintain up-to-date clinical knowledge and adhere to CQC, GDPR, and NMC standards.
- Close treatment cycles: Audit and finalize all documentation and systems for completed cycles.

What key skills do I need to apply:

To be successful in this role you will need to have the following skills and attributes:

- Registered General Nurse (RGN)
- SACT qualification
- 3+ years post-registration experience
- Extensive chemotherapy nursing background
